What to send
What helps us book the right visit
The reason
The tooth or area, and what you would like us to look at or do.
Films and charting
Recent radiographs and any periodontal charting you have, sent through the secure form.
The patient's contact details
A phone number and the best time to call. We call the patient the same week.
Your plan for the next step
If a restoration is waiting, tell us which tooth and what it is.
The referral desk
A person answers, not a menu
The desk books the nearest of our three offices and tells you the date. If a patient calls us first, the desk asks who their dentist is and sends that office a note.
One phone number for all three offices
Hours: Monday to Thursday, 7:30 a.m. to 5 p.m.; Friday until 1 p.m.
A written report to the referring office

Questions
From referring offices
Can the patient call us instead?
Yes. Patients can call the referral desk themselves, and the desk asks who their dentist is so we can write to you.
Do you send the patient back?
Yes. Your patient returns to you for routine and restorative care. We see them for gum maintenance only if you and the patient agree.
Which insurance do you take?
Many plans, and it depends on the plan. The referral desk checks a patient's benefits before the first visit. See How fees work.
What about an urgent referral?
Call the referral desk and say it is urgent. We will do what we can, and we do not promise a day. For swelling that spreads to the face or neck, or trouble breathing or swallowing, send the patient to an emergency room or call 911.
Prefer to talk it through?
Call the referral desk at (413) 555-0100.
